This will be located under the “Forms” menu from the main screen. From there, use your mouse and drill down to 3 – Incident Report” and fill out the appropriate information.

The Uniform Specification Manual can be found within the Fire Manager System by logging on with your account and clicking on “Storage.” Once there, click on Storage – Employee Manuals – Uniform Specification Manual. If you sustain an on-duty injury, your Company Officer needs to be made aware immediately and he/she will contact the Battalion Chief. You will then need to log into Fire Manager with your account and click on “Forms – 3 Incident Report”. This needs to be done by law within 48 hours from the time of the incident so Workman’s Compensation will cover the injury. The rest of the process will be explained to you as needed.
The WRTFD provides the Employee Assistance Program (EAP) provided by Franciscan Network. EAP is there for you if you need help with any issue including spousal, personal mental health issues…etc. You can find all the information on the Fire Manager System by logging in and clicking “Storage – Employee Benefits – EAP”. All you need to do if look at any of the documents, make the phone call and show up. The Fire Chief DOES NOT receive any information about you or anyone in your family if you are attending EAP. The Department only receives the number of individuals who are using the program. Please do not be afraid of getting help for any issue that may creep up in your life.
You can log into the Fire Manager System and click on “Forms” and in that list, you will see separate forms for Equipment and Apparatus issues by station. Make sure you choose the correct form and the correct station. The form once submitted will be sent via email to the appropriate Station Captain for review before sending it to Chief Skipper. This way assists Chief Skipper in “weeding out” duplicate entries for the same problem.
